Membership Requirements
To ensure the highest level of quality, the CAP Board of Advisors established formal membership requirements. These requirements are intended to establish a solid foundation for clinical integration and demonstrate commitment to a high standard of quality of care.
CAP Physician Requirements
Members must:
- Maintain an Affiliate Physician Appointment at Columbia University Irving Medical Center by meeting all applicable annual requirements.
- Meet all requirements as established by the CAP Credentialing Program, including:
- Initial credentialing, re-credentialing, and ongoing monitoring
- Maintain and submit copies of current and valid credentialing documents annually
- Ongoing Professionalism: Members are expected to conduct themselves in a manner which promotes the values and behaviors associated with (a) Columbia's organizational climate and culture that encourages civility, acceptance of differences, respect for all, ethical behavior, and a supportive environment within our community and (b) Columbia's commitment to trust, equity, teamwork and a shared responsibility to model professionalism.
- Maintain active privileges at an NYP affiliated hospital in cases where geography warrants such an affiliation, unless waived. At this time, this applies only to NYP/Lawrence and NYP/Hudson Valley Hospital for Westchester providers.
- Attend at least one clinical departmental meeting or grand rounds (may be web-based). Note: your sponsoring department may have more stringent requirements.
- Show evidence of meaningful collaboration with ColumbiaDoctors physicians in diagnosing and treating patients. Practice medicine according to the requirements of the relevant ColumbiaDoctors Clinical Department.
- Meet the quality standards, as they are developed, of the ColumbiaDoctors Division of Quality and/or the relevant ColumbiaDoctors Clinical Department.
- Actively utilize an ONC Certified Electronic Health Record or Health Registry. Participate in the Medicare Merit-Based Incentive Payment System or an approved alternative payment model and other governmental programs where eligible.
- Participate in relevant quality bonus programs offered through CAP, which may involve patient data collection.
- Verify that your practice is compliant with Managed Care Program Access and Availability standards and provide CAP with current practice and billing information.
Annual Attestation
As part of the annual attestation, physicians are required to:
- Complete a CAP Membership Requirements Attestation form.
- If you have not fulfilled a requirement, please attach a letter to the CAP Board of Advisors explaining the reason behind the non-compliance or inability to comply within the required timeframe.
- Complete a CAP Credentialing Disclosure and Authorization, Attestation, and Release Form.
- Must include an explanation for any “yes” answers.
- Provide required information regarding your electronic health record (EHR) or registry via an online form.
- Submit copies of current and valid credentialing documents, including:
- IRS W9 form–for each tax ID
- State License Registration Certificate–for each practicing state
- DEA/CDS Certificate–for each practicing state
- Board Certification–for each specialty
- Hospital Privileges Appointment Letter–for each active affiliation
- Malpractice Liability Coverage Face Sheet–$1 million/$3 million minimum
- Infection Control Certificate
